Request a Letter or Form
- All requests for forms are to be made to reception, via phone.
- The receptionist will advise you as to whether the form will require a doctors visit - if so this will be scheduled for you at the next available time.
- There will be an administrative charge for all forms - dependant on the time required by the doctor to complete the form.
- All Legal forms are to be requested by your solicitor.
- All PIAB Reports are to be requested in writing by the patient if not dealing with a solicitor.
- Please provide your forms well in advance of the date you require them as only a limited number of forms can be processed by a doctor in any working day. With this in mind it is not possible to return forms in less than 48 working hours-PIAB & Medico Legal requests will take between 4-6 weeks.

Charges for Medical Card Holders
There are additional services that are NOT covered by the General Medical Services Card. Many of these are outlines below. A request for one of these additional services may incur a fee. Please check with us before requesting one of the services to avoid any misunderstanding in relation to fees.
- Insurance policy certificates and medical examinations
- Examinations and reports for solicitors;
- School attendance notes and certificates;
- Travel, work-related and pregnancy vaccinations;
- Phlebotomy (the taking of blood tests);
- Passport and ID reports;
- Letters to councils, creches and general advisory bodies;
- Sports medicals;
- Drug Kardaxes for nursing homes and other residential homes;
- Letters in support of social welfare / council applications, etc;
- Driving licenses;
- Power of Attorney and Enduring Power of Attorney reports and certification;
- Examinations for mental capacity to make a will;
- Certification of retrospective mental capacity;
- Certification for insurance companies;
- Private Medical Attendant's reports / consultation / examination;
- Medicals and health checks;
- Travel insurance reports;
- Certificates of fitness to travel;
- Certificates of fitness to drive;Certificates of fitness for Gyms, Scuba Diving, Sport, etc;
- Certificates of fitness to work, or to return to work, or paternity leave;Examinations and treatment following an accident, performed for legal purposes;
- Examination for solicitors reports;
- TUSLA reports;
- Adoption and foster care medical and certification;
- Examinations requested by public hospitals or private consultants including but not limited to: blood tests, breathing tests, heart monitors, etc;
